This article is devoted to formalization the task of information support in decision making during diagnosis and treatment of primary open-angle glaucoma for mathematical models synthesis, which intends for determination of primary open-angle glaucoma severity and the treatment approach. The structure of the network model for stage definition and treatment approach choice of primary open-angle glaucoma was developed. It’s allowed to raise a certainty and validity of decisions at the diagnosis stage and treatment action stage.
